  1. In a medium saucepan, combine the granulated sugar, whole milk, cocoa powder, and a pinch of salt. Stir until well combined.
  2. Place the saucepan over medium heat and bring the mixture to a boil, stirring constantly.
  3. Once it reaches a boil, stop stirring and allow it to boil for about 5 minutes without stirring, or until it reaches the soft ball stage (about 234°F or 112°C on a candy thermometer).
  4. Remove the saucepan from heat and add the unsalted butter and vanilla extract. If using, add the chopped nuts as well.
  5. Let the mixture cool for about 5 minutes without stirring.
  6. Using a wooden spoon or a hand mixer, beat the fudge mixture until it thickens and loses its gloss (about 5-10 minutes).
  7. Pour the fudge into a greased 8×8-inch square baking dish and spread it evenly.
  8. Allow the fudge to cool completely at room temperature before cutting it into squares.
